Veterans RAND 12 Item Health Survey (VR-12 )

The VR-12 developed from the VR-36 and MOS SF-36 Version 1.0

The VR-12 is self-administered and more than 5 million administrations have occured in the Veterans Administration, Medicare and other settings. 

The assessments have been implemented in many ambulatory patient populations and the community including those diagnosed with cancer. The assessment spans the physical to the psychological domains with as few as 10 items to create a physical and mental summary.

VR-12 also includes two “extra” items in order to assess how much physical and emotional health has changed over the past year.
